Practice Subsidy Claim Form - Instructions
NB Practice Subsidy Claim Form only applies to GP Term 1 and 2
We have been able to simplify this form and reduce the amount of data required for 2010
Number of patients – The number of patients seen that week
Hours worked in the practice – Total time for consulting, phone calls, results, reports, teaching, and visits (hospital, nursing home, home visits, other institutional visits etc) but exclude after-hours on-call work. Effectively this is the total time worked each day from start to finish, less any true lunch break, and excluding after-hours on-call work.
Hours of external workshops – Time spent attending Valley to Coast workshops. Count a full day workshop as 8 hours.
Total Hours – This should be the sum of the previous two columns. This will be the same as the number of hours that you are paying the registrar for (other than after-hours on-call work which will be extra).
Leave - enter any leave taken by the registrar during this period. Also enter any leave taken by the registrar’s GP supervisor and note the replacement supervisor.
Practice Subsidy claim for this period
Note that Practice subsidy is paid as a set amount for each week of the term, regardless of whether the registrar is working or on leave.
